What is the policy for Prescription refills?

We do ask for a 24 hour notice for any prescription refill. If you are requesting more refills on a prescription from a local pharmacy, we require you to pick up a hand written prescription. If your pet is on a medication for a chronic illness such as Hyperthyroidism, Renal Failure, or Heart Disease, or if they are on an NSAID for arthritis, we require bloodwork every 6 months to ensure the medications are working like they should.

Do you offer payment plans?

We do not offer payment plans. But we do accept CareCredit® and ScratchPay® at our office which offer convenient payment options. Visit our payment page to learn more about these options.

Do you see Exotic Pets?

Doctor Perzak does treat small mammals such as Guinea Pigs, Hamsters, and Rabbits. We do not treat amphibians, birds, reptiles or fish. Do you go by appointments?

We do go by appointments to try and minimize your wait time. An appointment is typically 20-30 minutes in duration. This allows the doctor ample time to perform an exam and answer any questions. If you require a Health Certificate, we schedule an hour to allow us time to make sure you have everything you need.

What are your vaccination requirements for Day Care and Boarding?

We require each pet staying with us to have a doctor/patient relationship. For Canines we require up to date Rabies, Distemper, Canine Influenza, and a Bordetella as well as a negative Intestinal Parasite (Fecal) Exam within the past six months. For Felines we require up to date Rabies and Distemper as well as a Negative Intestinal Parasite Exam within the past six months.
